Collect membership fees

The following example shows how to configure Tamaro widget to collect membership fees:

Only recurring payments with a semestral and yearly intervals are enabled.
The amounts list is being dynamically changed based on the selected interval (using conditions).
Some unneeded blocks and fields are hidden.
Custom 'stored_customer_accept_terms' field is added.
Some translations are adjusted.
